Dream Child Eligibility Guidelines
Children between the ages of 4 and 19 who reside
in the Dream Come True service area (for a minimum of six months)
and who have a terminal illness, or who have an illness or chronic
condition that has significantly altered their quality of life.
The child must be able to derive enjoyment from a dream and must
be able to articulate a dream.
How to Become Eligible
Once the letter is received by Dream Come True,
an authorization form will be mailed to the parents. This form will
explain the mission of Dream Come True and ask for authorization
to contact the child's physician to verify the diagnosis. NOTE:
a child is deemed eligible only after verification is received and
approved by the Dream Come True board of directors.

The child must be referred to Dream Come True in
writing or via the on-line form. Anyone
may refer a child (e.g. doctor, mother, teacher, friend, etc.).
The letter must state the child's name, age, address, diagnosis,
parents' names, address, and telephone number.
Dream Ineligible Children
Severe brain damage, irrespective of the primary
diagnosis. Severe being described as "incapable of enjoying a dream"
as determined by a physician or trained professional. Short term,
completely treatable conditions. Children with primary diagnosis
of Cerebral Palsy. These children will be referred to other appropriate
organizations.
